Insurance Tax Returns and Installment Reporting Statements
 
The tax forms for 2011 are now available. Effective October 1, 2010, the premium tax rate for non-A&H business was changed from 1.7% to 2.0%. The forms have been updated and simplified, including combining the Property and Casualty Form and the RRG Form and combining the HMO Form and the GHMSI Form.  The District of Columbia plans to allow the submission of premium tax returns and payments through the NAIC OPTins service.  If you have any questions, please contact Philip Barlow, Associate Commissioner for Insurance at (202) 442-7823 or philip.barlow@dc.gov.
 
Instructions: Select the appropriate tax form, type in the information directly on it. Make sure to complete all the required fields. Print out a copy for your records. Sign the tax return and mail it to the address indicated at the bottom of the form. (Please note: if you owe money, you must send your form and payment by OPTins or mail your payment along with a copy of the return to the Lockbox address listed on the form.)
